Aboriginal Mental Health Liaison (Fremantle Hospital)

Aboriginal Mental Health Liaison (Fremantle Hospital)

The Aboriginal Mental Health Liaison service provides cultural expertise and culturally safe care to Aboriginal and Torres Strait Islander consumers accessing mental health services at Fremantle Hospital.

The service works across both inpatient and outpatient community mental health teams within the Fremantle Hospital catchment. The SMHS mental health catchment map (PDF 262KB) lists suburbs in this catchment area.

The team includes male and female Aboriginal staff to provide a culturally appropriate service for both men’s and women’s business.

About our service

Our service provides cultural input for:

  • inpatient admission assessment
  • discharge planning meetings
  • family meetings
  • case managed clients on community teams
  • doctor’s consults
  • Mental Health Review Tribunals
  • State Administrative Tribunal hearings.

If you are discharged within in the catchment area, ongoing support can be accessed alongside your Care Coordinator within the community setting through Outpatient Services at Alma Street with appropriate referral.

Referral process

Self-referral

New or existing Aboriginal consumers accessing mental health services can ask their doctor, nurse, or care coordinator to arrange an initial ‘meet and greet’ with an Aboriginal Mental Health Liaison.

Health professionals

Your hospital doctor, nurse, or care coordinator can also refer you to our service. Referring health professionals are asked to include relevant information required to assess their client’s needs.
