Under the Freedom of Information Act 1992 (external site) you can access your medical records and other documentation held by SMHS, subject to some limitations or exemptions. Freedom of Information is also known as FOI.

Each SMHS hospital manages its own patient records

To access your medical records, you will need to contact the hospital where you were a patient.

Each SMHS hospital has an FOI Coordinator and your request must be received in writing, either by submitting a:

• formal letter of request, or
• completed FOI application form.
