Mental Health Crisis Centre

The Mental Health Crisis Centre (MHCC) is the centralised point of access to mental health services offered at Fremantle Hospital.

The centre provides specialist multidisciplinary community mental health assessment and treatment for people aged 18–65 years with mental health issues who live in the Fiona Stanley Fremantle Hospital Group (FSFHG) catchment area.

Our outpatient multi-disciplinary team consists of:

  • nurse practitioner
  • mental health nurses
  • occupational therapist
  • social workers
  • senior mental health professionals
  • Lived Experience peer workers
  • Aboriginal mental health liaison
  • welfare officer
  • consultant psychiatrist
  • health service medical practitioner
  • psychiatric registrar.

Referral methods include self-referral, family member/friend, GP or other service provider.

How the service works

When you first contact our service in person or by phone, a clinician will complete an initial assessment of your mental health and direct you to appropriate pathways to care.

If appropriate, you may be referral to our community mental health service which will work with you, your family and your GP to manage your care. Your personal aims and goals will be incorporated into this care.

The centre provides face-to-face and virtual assessments and can conduct community mental health assessments as appropriate. It works alongside the Police Co Response and Ambulance Co-Response services to provide timely mental health assessment and treatment.

Our service is a smoke-free environment. Please note smoking is not permitted in our buildings or grounds as per the Department of Health’s smoke-free policy.

How to access our services

Self-referral

You may self-present for this service or make contact via the hospital switchboard on 9431 3333.

Health professional referral

You can also be referred to the centre by:

  • your general practitioner (GP)
  • other hospitals and emergency departments
  • psychologists
  • community managed organisations.
Health professionals are asked to complete the referral form (Word 148KB) and return to the email address provided on the form.

After we receive your referral

After receiving your referral, a triage clinician will phone you to discuss your needs and determine which service is most suitable for you.

You may be offered follow up with the Assessment Treatment Team, or if it appears our services are not appropriate to your needs you will be referred to your GP or signposted to the most appropriate service.

We welcome your feedback

The centre service welcomes compliments, complaints and suggestions to assist in evaluating and improving its services.

Feedback can be provided at any time by:

  • asking an MHCC team member for a feedback form
  • completing an online feedback form (external site)
  • phoning the Fremantle Hospital Patient and Family Liaison Service on 9431 2787.
