Older adult inpatient and community mental health referrals

Older adult inpatient and community mental health referrals

Fremantle Older Adult Mental Health Service (FOAMHS) triage is the point of access to community services for older adults over 65 years who reside in the Fiona Stanley Fremantle Hospitals Group catchment area (refer to the SMHS Mental Health catchment map (PDF 262KB).

The service links with inpatient mental health services.

Health professionals referring to FOAMHS are asked to either:

  • complete an organic screen using the referral checklist and flowchart (PDF 195KB) and the relevant referral form (PDF 43KB) and fax to 9431 3619.
  • or fax appropriate information to facilitate follow up.

Each referral will be reviewed by the FOAMHS Triage Officer and presented to the next FOAMHS intake meeting (Monday to Friday). Faxes received after hours will be processed on the next business day. The referrer and consumer will be contacted to discuss the outcome and proposed action plan.

After hours referral can be made via the Assessment and Treatment Team (ATT) Triage Team.
