Peel and Rockingham Kwinana

Peel and Rockingham Kwinana

Peel and Rockingham Kwinana (PaRK) Mental Health provides health services to adults living in the communities of Kwinana, Rockingham, Mandurah, Pinjarra and Waroona.

Adult Services (age 18 – 64)

Peel and Rockingham Kwinana Mental Health provides services to adults with a primary diagnosis, or suspected diagnosis, of mental illness who require specialist mental health care.  

All referring health professionals are asked to complete the relevant referral form and fax to the appropriate site:

  • Peel Mental Health Service referral form (PDF 197KB)
  • Rockingham Kwinana Mental Health Service referral form (PDF 198KB).

Each referral is reviewed by the Triage Officer and presented to the multi-disciplinary team. The referrer and the referred client will be contacted to discuss the outcome and proposed action plan.

General Practitioner Liaison Service (Peel and Rockingham Kwinana Mental Health)

The General Practitioner Liaison (GPL) Service for Peel and Rockingham Kwinana provides case conference and clinic assessment services to GPs treating adult patients aged 18 to 65 years.

Older Adult Services (age 65+)

The Older Adult Community Mental Health Service provides specialist mental health services to individuals 65 years or older, or 45 years or older for Aboriginal consumers who are either experiencing a mental illness or whose mental disorder is complicated by physical and cognitive conditions associated with ageing.

All referring health professionals are asked to contact the Older Adult service (Peel and Rockingham Kwinana: 6557 4900) to obtain a copy of the referral form.
