YouthReach South
YouthReach South aims to improve the function and quality of life for young people with complex mental health needs who live in the south metropolitan and sections of the east metropolitan areas. The YouthReach South Triage Officer can advise on areas of inclusion.
YouthReach South provides counselling, therapy and care coordination to young people aged 13 to 24 years who experience barriers to accessing mainstream services.
These barriers typically include:
- homelessness
- limited support networks
- cultural barriers
- marginalisation due to diverse sexuality and gender.
The welcoming, trauma-informed, and flexible approach of this service supports young people during the assessment and treatment of their mental health issues.
Services
YouthReach South offers the following:
- assessment
- intensive care coordination and psychosocial support
- assertive outreach
- evidence based counselling, psychotherapy and psycho-education
- transitional support
- consultative advice and support to other agencies including collaborative clinical interventions and projects
- in-reach to the statewide homeless youth facility Ngatti House, including:
- care coordination
- comprehensive psychiatric care
- prescription and management of medication
- metabolic screening
- depot injection administration.
These services are provided by:
- clinical psychologists
- social workers
- clinical nurses
- occupational therapists
- Aboriginal mental health workers.
Psychiatry services are also available.
Clinical services are delivered in a variety of settings to best support the young person’s needs and/or stage of engagement, including:
- home
- community organisations
- inpatient settings
- the service’s offices.
Referral information
Referrals are accepted from a variety of sources, including:
- patients and family members
- health and community-based professionals.
