Youth Reach South referrals

YouthReach South aims to improve the function and quality of life for young people with complex mental health needs who live in the south metropolitan and sections of the east metropolitan areas.

The YouthReach South Triage Officer can advise on areas of inclusion.

Referrals are accepted from a variety of sources, including:

• patient and family
• community members
• community managed organisations
• mental health services
• general practitioners
• emergency departments
• private practitioners.

Referrals and enquiries can be made by:

• phoning 9431 4700 between 8.30am and 4.30pm
• completing the referral form (Word 101KB) and emailing to YouthReach South Triage.
