Assessment and Treatment Team

The Assessment and Treatment Team (ATT) is a short-term community mental health team providing specialist mental health assessment, care coordination and intervention for adults aged 18-65 who live in the Fiona Stanley Fremantle Hospital Group (FSFHG) catchment area. Refer to the SMHS mental health catchment map (PDF 262KB) to see suburbs that fall within the FSFHG catchment.

Referrals are received via the Mental Health Crisis Centre (single point of access into the service) and Fremantle Mental Health inpatient services. The team work with consumers for up to 12 weeks of community-based treatment and recovery support.

Our multi-disciplinary team consists of:

  • clinical nurse specialist
  • clinical nurses
  • occupational therapists
  • social workers
  • clinical psychologist
  • consultant psychiatrist
  • psychiatric registrar.

How the service works

The service is open from 08:30am to 4:00pm, Monday to Friday. 

After receiving your referral, a clinician will phone you to discuss your needs and determine if our team is suitable for you. If it appears our services are not appropriate to your needs you will be referred to your GP or signposted to the most appropriate service.

Your allocated care coordinator will arrange a time to meet with you to develop a recovery focused Treatment Support and Discharge Plan that incorporates your personal values and goals. The team will work with you, your personal support persons, and your GP for a time limited period to support your recovery journey. 

Most of your treatment time will be supported by your care coordinator. To always ensure your safety and wellbeing, you may also have contact with other clinicians who will work collaboratively to offer you the most appropriate service. You will work with your care coordinator to achieve your recovery goals.

The ATT can see you out in community or in the clinic at Fremantle Hospital. 

Our service is a smoke-free environment

Please note smoking or vaping is not permitted in our buildings or grounds as per the Department of Health’s smoke-free policy.

If you need continuing support

If you need support beyond the ATT’s 12-week care period, we will refer you to a service which may include:

  • another specialist mental health team
  • a community managed organisation
  • your GP.

How to access our services

If you would like to access the ATT we recommend you ask your general practitioner (GP) for a referral to the Mental Health Crisis Centre, which is the single point of access to the service.

The ATT service welcomes compliments, complaints and suggestions to assist in evaluating and improving its services.

Feedback can be provided at any time by:

  • asking an ATT team member for a feedback form
  • completing an online feedback form (external site)
  • phoning the Fremantle Hospital Patient and Family Liaison Service on 9431 2787.
